Our volunteers provide a wide-range of support across different wards, departments and services in our hospitals and are often the first point of contact for people when they arrive. They help our staff too, and their support makes a big difference. 

Our Volunteer Award recognises our wonderful volunteers who work tirelessly and generously give up their free time to help improve our services and support our staff, patients and visitors.

Please include how your nominee demonstrates or has demonstrated
one or more of the following in your nomination.   

•  Regularly contributes to the wider NUH team        
•  Consistently demonstrates a positive attitude                      
•  Has a significant impact on patients, staff or the services they support

•  Is a role model for volunteering 

Please note: Nominations for this award can be submitted by staff, patients or any member of the public.

What we do with your information

What you should not submit via our form

Each question within our form clearly stipulates the information required from you. Please do not include further details about yourself or anyone else within this form.

What information do we collect?

  • Your name and email address.
  • The name of your nominee, their job title if applicable and their work location.
  • The reason for your nomination.
  • Your permission for the nominee to know you nominated them.

How we use your information

Your personal data gathered in this form will only be used to contact you if:

  • Your nomination is not complete or has not been submitted correctly.
  • The person/team you have nominated has been shortlisted.

    None of your personal information is sold on or shared with any third party organisation.

Who can access your information

  • NUH Communication Team members only. This access is required to confirm your nomination and to keep an accurate record of nominations.

How we store your information

  • The form is securely downloaded and saved within our protected hospital computer network. 

How long we store your information for

  • We will delete the data gathered after 12 months.

Removal of your information from our records

  • You are entitled to request that we remove any personal information we have about you stored within our systems at any time.
